FAST
Registration Form--This is the form you need to fill out to become a member
of FAST and pay your dues. (Required)
Medical Form--This is the form
you need to fill out to give us medical information about your swimmer (Required)
USA
Swimming Registration--This is the form to get registered with USA Swimming
and be placed on FAST's official charter roster. (Required)
You cannot swim in any meets without USA Swimming membership.

Swimming Non-Athlete Registration--This form is for Coaches, Officials, Meet
Directors, and other ADULTS who are working with the team in an official
capacity.
